Thanks for your question!  The cart works much like it would for any shopping website.  If you look at your cart and see the list of items in it, there should be a two buttons towards the bottom of the screen.

One button should say 'continue browsing' - you would click that if you had other things you wanted to get. But the one you want says 'proceed to checkout'.  After clicking that, you get one other screen that says 'confirm checkout'.  Then you'll be taken to the Download screen.

Since you have the Kindle you should have chosen the Kindle format for your book, so the button on your download screen should say 'Get for Kindle'.  When you click this button you should be transferred to Amazon's web site, to the page where you can send the book to your Kindle.

You might have to log in to Amazon before it will let you continue.  But once you get to Amazon's site, the process is very similar to actually purchasing a book through Amazon.

Hopefully this gets you there and the book will be on your Kindle the next time your Kindle syncs.  Please let me know if you have any problems or other questions.
