While I can't quite give you a definitive answer here, I would suggest contacting the doctor's office or hospital where you had these tests done. They would probably be the place most likely to have the records or at least be able to point you in the right direction. I could certainly find a phone number or email address for one of those places if that would be helpful.
I have attached links to the relevant laws around medical records in Nebraska (if that is where your records are), but here is a brief summary: Hospitals are required to keep most medical records for 10 years, but doctors working elsewhere do not have the same requirement. You will be asked to submit your request in writing. The health care provider then has 30 days to provide your records. They are allowed to charge up to $20 as a handling fee and no more than fifty cents per page for regular paper. They can charge more for X-rays because of the cost of the materials and the labor associated with it.
